Renewal Process in Michigan State

Before Your Online Consultation:

  1. Register and complete your application at FadeMD.com.
  2. Schedule an appointment with one of our licensed physicians.
  3. At the time of your appointment, log into your FadeMD account to enter the waiting room. Ensure that your camera and microphone are enabled.

After Your Online Consultation:

  1. Visit the Michigan State website. Please go to:  www.Michigan.gov/CRA then go to 'patients&caregivers' at the link here: https://www.michigan.gov/cra/sections/mmp where you have the option to register or log in.
  2. Navigate to 'My Records', click the small triangle next to 'Registry Cards', then select 'Renew Application' and proceed by clicking 'Continue Application'.
  3. Confirm your address details and click 'Continue Application'. Next, select your proof of residency and review your name and date of birth.
  4. Choose from the dropdown menu whether your physician will certify you online or if you need to upload a scanned copy of your FadeMD paper certification.
  5. Enter the Michigan Physician License Number that is listed on your FadeMD certificate.
  6. Provide your payment details and click 'Pay Now'.
  7. Your application will be processed within 15 business days.

Note: After your application is processed, you will receive an email from noreply@accela.com. Be sure to check your spam or junk folder. You can monitor the status of your application at any time by logging back into the site and checking under 'Records'.
