Michigan State Process

Before your tele-visit:

1. Register and complete your application at FadeMD.com

2. Choose an appointment time with one of our licensed physicians. 

3. At the time of your appointment, login to your FadeMD account where you will join the waiting room. Please make sure to enable your camera and microphone.

After your tele-visit:

1. Visit the Michigan State Website and click on 'Register For An Account'

2. Check the 'General Disclaimer' box > 'Continue Registration' > Fill out all required fields.

3. Once your information is filled in, it will take you back to the home page and you may now login at https://aca-prod.accela.com/MIMM/Default.aspx

4. After you have logged in, click on the 'Registry Cards' tab. > Accept general disclaimer. > Click on 'Registry Cards' dropdown and select 'Patient Only Registration Application' > 'Continue Application'.

5. Verify your address and choose what proof of residency you will use. 

6. Select from the dropdown whether your physician will certify you online or if you will upload a copy of the paper certification.

7. Complete the required fields and enter your payment information. 

8. Once you have completed your registration, you will receive a confirmation email. Your doctor will be notified and has up to 15 days to complete their portion. Once this is complete, you will receive an email from noreply@accela.com. Please also make sure to check your spam/junk folder.

9. You will receive your medical marijuana card in the mail.
