Process in the State of Michigan

Before Your Online Consultation:

  1. Enroll and complete your application at FadeMD.com.
  2. Schedule a time to meet with one of our certified doctors.
  3. Log into your FadeMD account at your designated appointment time and enter the waiting room. Be sure your camera and microphone are enabled.

After Your Online Consultation:

  1. Go to the Michigan State Website and initiate registration by selecting 'Register For An Account.'
  2. Agree to the 'General Disclaimer,' proceed with 'Continue Registration,' and complete all mandatory fields.
  3. After entering your information, return to the homepage and log in at Michigan Medical Marijuana Program.
  4. Once logged in, navigate to the 'Registry Cards' tab, accept the disclaimer, and from the dropdown menu, choose 'Patient Only Registration Application.' Click 'Continue Application.'
  5. Confirm your address details and specify your proof of residency.
  6. Decide whether your physician will certify you online or if you need to upload a physical certification.
  7. Fill in the necessary information and submit your payment details.
  8. After completing your registration, watch for a confirmation email. Your physician has up to 15 days to complete their part of the process. Confirmation of this will be sent from noreply@accela.com. Check your spam/junk folder as well.
  9. Your medical marijuana card will be mailed to you once all steps are completed.
