Application process in the State of Illinois

Prior to Your Online Consultation:

1. Sign up and fill out your profile on FadeMD.com.
2. Book a session with one of our certified doctors.
3. Before your scheduled session, ensure you're logged into your FadeMD account. Join the virtual waiting room, making sure your video and audio are activated.

Following Your Online Consultation:

1. Visit the official Illinois State website, navigate to 'MCPP Patient Registration', and select '+New'. Your certificate should appear in the dropdown menu for you to select. Fill out the necessary fields and click 'Register'.
2. Look out for an email that provides a temporary password. You will have an opportunity to update this password once you log in.
3. Select '+New MCPP Patient Registration' once more and input all required details.
4. For your digital signature, enter your name and hit 'Save'.
5. Under the 'Provider Certification' section, you'll need to upload your certificate. Typically, receiving your certificate takes 24-48 hours post-approval, except if you've selected the expedited option.
6. Make your payment.
7. Your application will be marked as 'SOS Check' until your card is fully processed. To print your provisional letter, which allows you to buy medical marijuana, click on 'Initial Application' followed by 'Provisional Letter' found in the 'Tracking Inbox'.
