Process in the State of Oklahoma

Before Your Online Consultation:

1. Complete your registration and application on FadeMD.
2. Select a time to meet with one of our qualified physicians.
3. Before your appointment, log into your FadeMD account to access the waiting room, ensuring your camera and microphone are active.

After Your Online Consultation:

1. After your certification is approved and uploaded, log into your FadeMD account and click on your email at the top right corner.
2. Navigate to "Order Pickup," select your most recent order, and download your temporary certificate from the provided link.
3. Go to the OMMA Access Portal, click 'Access Portal,' then 'Sign Up Now.' Enter a valid email address and request a verification code.
4. Retrieve the verification code from your email, enter it, and click 'Verify'.
5. Select 'Create a New Profile,' fill in your details, then choose 'Apply For a New License' and complete the necessary forms.
