Application Process in the State of Ohio

Before Your Online Consultation:

  1. Sign up and fill in your application at FadeMD.com.
  2. Schedule a consultation with one of our qualified physicians.
  3. When it's time for your appointment, log into your FadeMD account to access the waiting room. Be sure to enable your camera and microphone.

After Your Online Consultation:

  1. Your doctor will complete your certification, and you'll receive a state-issued email within 24-48 hours of your consultation.
  2. Use the link in the email to go to the state registration site and set up your profile. Remember, this link can only be accessed once, and your email address must not be linked to another account. If it is, you'll need a different email to continue.
  3. Check that all information your physician entered is accurate. If you notice any mistakes, please contact our support team.
  4. Proceed to 'Activate Card' and submit the registration fee. You can then download and print your card.

Important Reminder: If you haven't received the state's email within 48 hours post-appointment, email MMCPRegistry@pharmacy.ohio.gov with your full name, birth date, and a working email address to request a new registration link.
