Process in the State of Missouri
Before Your Online Consultation:
- Sign up and fill out your application on FadeMD.com.
- Select an appointment time with one of our licensed physicians.
- Log in to your FadeMD account at the appointed time to enter the waiting room, ensuring your camera and microphone are on.
NEXT STEP: Register with the State of Missouri (Complia Portal) to finish your application.
IMPORTANT (30-DAY DEADLINE):
Missouri requires your state application to be submitted within 30 days of the signature date on your Electronic Certification. If you miss this window, your application may be rejected and you may need a new certification.
Step 1 — Go to the Missouri “How to Apply” page:

Step 2 — Click “Patient/Caregiver Application Registration”
(If a “Warning – Exiting Site” pop-up appears, click “Yes” to proceed.)

Step 3 — Complete your application in the portal
After signing in, click “+Create New Application” and choose “Patient” / “New Patient Registration” (if you are new).
Step 4 — Select your Electronic Certification (this is important)
When you reach the “Physician/Condition Information” tab:
• Under “Recommendation Type,” select “Electronic Certification”
• Click “View Available Certifications”
• Choose your certification and click “Update”

What happens next:
• Once you submit your application, the state processes it in the order received. (Processing times can vary.)
