Process in the State of Missouri

Before Your Online Consultation:

1. Sign up and fill out your application on FadeMD.com.
2. Select an appointment time with one of our licensed physicians.
3. Log in to your FadeMD account at the appointed time to enter the waiting room, ensuring your camera and microphone are on.

After Your Online Consultation:

1. Proceed to register on the state website as follows:a. Click 'Register' at the top right corner of the page.b. Complete the form with your contact details and Social Security number, which your doctor will need to upload your certificate.c. Agree to the terms & conditions.d. Finish the registration process.
2. For detailed instructions on how to complete your registration, refer to the step-by-step guide available here:Missouri Patient Registry User GuideEnsure on page 12 that you select “Electronic Certification” and not “Manual Certification.”
3. Once you pay the $25 state fee, you will receive your digital medical card.
