Application Process in New York State

Before Your Online Consultation:

  1. Register and fill out your application on FadeMD.com.
  2. Schedule a session with one of our certified physicians.
  3. Log in to your FadeMD account at the scheduled time to enter the waiting room. Make sure your camera and microphone are activated.

After Your Online Consultation:

  1. After receiving confirmation that your certificate is ready, log back into your FadeMD account and click your name in the top right corner.
  2. Navigate to "Order Pickup," select your most recent order, and download your certificate from the provided link.
  3. Visit the state website at New York State Login and select "Don't Have an Account?" to create a new account.
  4. Once your account setup is complete, log in again.
  5. Select the "Health Applications" tab.
  6. Access the "Medical Cannabis Data Management System (MCDMS)" by clicking on the appropriate link.
  7. Start your registration by clicking on "Start New Registration" and follow the instructions provided.

Please Note: Patients will be issued a temporary registry identification card that can be used, along with a government-issued photo ID, to purchase medical marijuana. The official card will be mailed to you.
