FadeMD provides a consistent rate of $99 for both initial certifications and renewals. In Pennsylvania, there is an annual state fee of $50, but this drops to $25 if you qualify for Medicaid, PACE/PACENET, CHIP, SNAP, or WIC.

For expedited certification, an additional $40 can be paid for quicker processing.
